The doctor comes out with a grim face to tell you that the rest of your life will be about a week longer. They can only make you comfortable until you pass away. Turns out you will be in debt for the rest of your life. They ask you to fill out a form so they can inform your next of kin that they will have to take on the debt that you leave behind.
